Hospital Health Information Management System (HHIMS)

Hospital Health Information Management System ( HHIMS) is an open-source medical database software designed for use in Sri Lanka hospitals. It stores clinical details of patients treated and is designed for use by clinical staff - details are recorded on the system as they examine the patient The system enables hospital staff to refer previous clinical records when the patient comes in for treatment, print visit slips or discharge letters for patients, print quarterly health statistics and prepare notifications of infectious diseases for the local Medical Officers of Health. The software will thus significantly reduce the need for maintaining paper based records and enable an improved and efficient service to the patient.
The HHIMS offers many advantages to higher level decision makers in the health care administration in Sri Lanka – it can help detect and control emerging and endemic health problems, monitor progress towards health goals, and promote equity in health care by empowering the citizen with timely and relevant health-related information, and drive improvements in quality of service. From the point of view of the policy maker it strengthens the evidence base for formulating effective health policies, permitting evaluation of scale-up efforts, and enabling innovation through research and improves efficient use of resources by way of effective stock controls. From a clinical perspective the HHIMS improves quality by giving timely access to important clinical information, mobilising new resources, and ensuring accountability in the way they are used.
Hospital Health Information Management System (HHIMS) is a free and open source platform introduced to manage the integrated care pathway of Sri Lankan health care institutions. Currently it has been piloted in two hospitals in the Kegalle district. The goal of the project is to improve the quality of care provided in government hospitals throughout Sri Lanka. This will be achieved by improved documentation of medical records using a reliable, easy to use, computerized record system. The system has been developed entirely on open source technology that can be used freely without license costs and is therefore affordable.
Helping decision makers to detect and control emerging and endemic health problems, monitor progress towards health goals, promoting the equity of care are key consideration in developing the system. HHIMS also strengthens the evidence for effective health policies, permitting evaluation of scale-up efforts, and enabling innovation through research and helping improve governance, mobilizing new resources, and ensuring accountability.
HHIMS has several modules to support the functions of the registration desk, doctor's desk, pharmacy and notification. Patient registration has a comprehensive electronic medical record including disease, symptoms, drugs dispensed, injections and laboratory investigations. This medical record system is the key component of the HHIMS facilitating effective access to past treatment records. The diagnosis complies with internationally accepted ICD 10 and SNOMED nomenclature. When the drugs are dispensed the system automatically shows the balance stock. HHIMS also automatically generates notification when a notifiable disease is recorded by the doctors. Apart from clinical details, the system provide various reports and statistics which can help hospital management and administration.

Public Health Midwives (PHM) in Sri Lanka is involved in preventive maternal and child healthcare activities on the field. The PHM maintains a number of routine documents to enable dissemination of data to regional and central levels. Traditionally these documents are maintained manually. It has been revealed that quality and timeliness of service delivery by the PHMs on the field is adversely affected by the heavy load of paper work necessitated in keeping these records and registers updated.
Praja Suwa Arunalu is a system that simplifies this cumbersome record keeping thereby giving the PHMs more time to involve themselves in on field healthcare activities. It also provides instant access to health information pertaining to the community served by them. Supervision, monitoring and evaluation of the field activities of the PHMs are also facilitated by this system. Another key advantage is that it is possible to generate several routine reports and returns that can then be escalated to regional and central levels. In addition it can store maps of the area and has the facility to mark index cases.
Maintenance of several registries is a part of the daily routine of Public Health Midwives (PHMs). They spend considerable amount of time on these tasks, which otherwise could be spent on servicing daily healthcare requirements of the community. The paper work involved is cumbersome and tedious - often repetitive and at times involving lengthy calculations.
e-Praja Suwa Arunalu has sought to address this problem and thereby make the daily routine of PHMs more efficient and effective. The light weight public health information system installed on a mini laptop is a powerful portable public health information tool, supplementing the PHM Diary and twenty odd daily registers. It also functions as a data aggregation tool for the District Health Information System, a WHO recommended public health reporting system.
Currently it is being piloted in the area under the jurisdiction of the Ratnapura Regional Director of Health Services and has rapidly gained popularity among PHMs involved. It helps PHMs to reduce paper work, and improve the data quality through semi automation of daily routines and calculations. e-Praja Suwa Arunalu is equipped with features to identify health problems and plan and track preventive measures, monitor and analyse grass root level data. It also has in built supervisory, monitoring and documentation tools to support higher level officers such as the Medical Officer of Health (MOH). Another noteworthy feature is the facility for digital mapping of households. The system addresses maternal and child health care as well as well woman care undertaken by the PHMs.

Sabaragamuwa Health Institutional Information Management System (SHIMS)

The SHIMS manages the core back end of the health institutional management process at a provincial level. There are four key modules – Pharmacy Information Management System, Bio Medical Information Management System, General Inventory Information Management System and Human Resource Management System. Three of the four modules have been developed and implemented across the hospitals in the Sabaragamuwa Province. While defining the policy and discipline for management, each of these modules facilitates stock/ resource management within the provincial system. The authorities concerned are also able to view the entire management path of any module in real time.
The Provincial Director of Health Services has the responsibility of managing all the state health institution under the authority of provincial government. This includes preventive health facilities as well as curative health institutions, like hospitals. The Office of the Provincial Director of Health Services has to manage the supplies of drugs and equipment, manpower and finances. The Sabaragamuwa Health Institutional Information Management System (SHIMS) seeks to provide the support of information technology to manage the essential services provided to the health institutions while monitoring the resource utilization in the provincial health service.
The system can take on board any number of medical institutions and can be adapted to the particular need of a given province. Moreover it has been designed such that it can be upgraded to run in cloud computing if so desired. Currently the SHIMS has been deployed in 130 medical institutions, including 105 hospitals in the Sabaragamuwa Province.
